Agreement to Re-direct Invoice to Marketer Form - To redirect your utility distribution bill to your marketer/agent.

Best Efforts Supply Request Form - To purchase gas supply service from the company on a best efforts basis.

Credit Transfer or Check Request Form - To request a gas transportation account transfer or refund for credit balance.

Customer Authorization to Release Information Form - To give a marketer/agent authorization to access your account information, such as consumption history.

Customer Pulse Service Request Form - To request pulse signals from your natural gas meter to monitor energy use on a real-time basis.

Customer Request Form - To change gas service elections. (For example, requests for transportation service.)

Demand Aggregator Authorization for Transportation Service Form - To assign indicated marketer as your exclusive agent for nominations, constraint notification and other pool functions associated with transportation service.

Marketer List - To get company names and phone numbers.

Telemetering, AC Power and Phone Line - To get telemetering requirements for transportation and interruptible sales.

Wisconsin Sales and Use Tax Exemption Certificate - To claim a tax exemption.
